Registration

There is a THREE PART registration process:

1. REQUIRED: Fill out the registration form via SurveyMonkey by clicking here
2. OPTIONAL: Fill out the group hotel form via SurveyMonkey by clicking here
3. REQUIRED: Pay the $75 registration fee (or obtain a fee waiver) by doing one of the following

• Mail a $75 check or money order US FUNDS ONLY to Let Them Hear Foundation, 149 Commonwealth Avenue, Suite 1014, Menlo Park, CA 94025
• Call 650 617-2253 and pay by credit card over the telephone
  If the registration fee is a hardship, call 650 617-2253 and request a fee waiver
• Parents/Patients, appointments WILL NOT BE made until the payment has been received or waived.
